Patient Admission Summary includes all admitted patients and active referrals.

Postponed or cancelled referrals may be included.

Discharged patients may also be included.

All columns within the summary list may be sorted both ascending and descending.

Once pertinent information has been entered, Face Sheets may be viewed in PDF format, if the patients LTC facility is HIPAA compliant, fax the Face Sheet upon admission.

Clicking on a patient's name will display patient information.

Referrals may be admitted, postponed or cancelled.

Payor Information, Medicare, Medicaid, Private Insurance.

Hospice Staff Assignments.

Pharmacy Selection, DME Provider Selection, Attending Physician Selection.

Nursing Assessment, Clinical Indicators, Hospice Plan of Care, Hospice Certification.

Medications Listing, may be faxed directly to pharmacy by RN from the patient's home.
